You should use both. A lead can be any contact and no doubt you have numerous ones which never pan out. Once you convert, it becomes an opportunity, this helps maintain your opportunity database and keeps forecasts more accurate.

Yes. A lead, is potential new business, which can also come from an existing company or customer in your CRM.

Same concept as web to lead.  we would customise case screen then generate HTML, your web guy would then create a web form using this rendered HTML. Anything entered into it would go straight to CRM under their name.
